I just signed up for a free account to www.SurveyMonkey.com after I discovered it through the zen-cart forums.

You  can design a multi-page survey to collect data and then later analyze it . The survey is hosted on their site. Easy form creation.

You can create (as I have done) a customer satisfaction survey to send out post transaction.

I'm thinking of placing a link to the survey on a business card to include in their package. Then, on the last page of the survey, I will thank them for their time and give them a coupon code to my site that isn't posted anyplace else. Sort of an incentive to take the survey.

I will tell them up front that the input they care to share about their shopping experience will be shared on my website in a testimonials section.

I see this as having great potential.

I know that in Joomla and other CMS /shopping carts, that you have a built in way to do surveys, but zen-cart does not. Neither does BISI or small websites with just paypal buttons. This is an easy, no-brain-required, way to get feedback on sites that otherwise aren't set up to collect it.

My business card that I will have the link on (or my follow up email to let them know their order has shipped) will first thank them for their order, and then tease them with something like "Want to earn a coupon for 20% off your next shopping spree? Take this short survey about your experience with JeriSigns.com".

not Jeri here ... but one of my kids teachers used this service to survey the parents a couple years back.
She seemed to really like it.  There are limits to how many and how often you can use it for free
sock monkeys at www.OurSockMonkey.com and of course everything at www.FilthyRichFashions.com

I used it for a little while and had great response/results. I don't remember why I stopped using it.

Yeah, the only snag for me (for the free version) is that you can only have 100 respondants per survey. But I created one last night, super easy to do.
